The CMAA exam includes 110 scored questions plus 25 pretest (unscored) questions, for 135 questions total. You won't know which questions are pretest, so answer every question as if it counts. The time limit is 2 hours 15 minutes.
What is the CMAA passing score?
NHA reports CMAA results as a scaled score rather than a raw percentage. A scaled score of 390 (on a 200–500 scale) is the widely published passing threshold set by NHA. Because NHA controls the exact cut score and equating, treat 390 as the benchmark and aim well above it in practice so you have a margin on test day.
How hard is the CMAA exam?
It's a moderately difficult, broad exam. In 2025, 63.24% of CMAA exam administrations passed (NHA 2025 annual report), which means about 36.76% of administrations did not result in a passing score. Most difficulty comes from breadth — you need working knowledge of medical terminology, HIPAA and compliance, scheduling systems, insurance and billing, and professional communication, all in one sitting.
